This is a major step toward simplifying life for Persons with Disabilities (PwDs) in India, or nothing. The old system was messy; you had to carry around piles of paper. The UDID portal changes that. It’s a single, unified system.
Here are the field notes on how the UDID (Unique Disability ID) Card application process works, which is the official route to get your disability certificate now.

UDID Card: The Single ID for PwDs
The UDID project is designed to create a national database and issue a single, universally accepted certificate and ID card. X was the old, scattered system. And then the UDID card followed, promising transparency and efficiency in accessing benefits.
📝 Step-by-Step Application Process
The whole thing starts and ends online, but the middle part requires a mandatory hospital visit.

Register Online: You must visit the official national UDID portal:
swavlambancard.gov.in. You register as a Person with a Disability (PwD) first. -
Fill Application: Log in and fill out the detailed application. This asks for your personal, identity, and disability details. This is where you upload any existing medical papers, if you have them.

Upload Documents (The Essentials): You need to scan and upload:

Photo and Signature.

Aadhaar Card.

Identity Proof and Address Proof.

Existing Disability Certificate/Medical Documents (if applicable).

Verification and Assessment: After you submit, the application is forwarded to the Chief Medical Officer (CMO) or District Medical Officer (DMO) of your district. The CMO/DMO verifies your application details. And then they refer you to a designated medical authority or specialist doctor(s) for the physical assessment.

Attend Assessment: You must go to the assigned hospital/medical board on the scheduled date. The specialist doctor(s) will assess your condition and determine the percentage and type of disability. This step is mandatory, let’s be real.

Receive Certificate/Card: Based on the medical board’s decision, the CMO office generates the Disability Certificate and the UDID Card electronically. You can usually download the e-certificate and e-card from the UDID portal after they are generated. The physical card is then dispatched to your registered address.

If the online process is too much, or nothing, you can get help. Contact your District Welfare Officer or District Social Welfare Officer. They facilitate registration counters and organize camps to help PwDs complete the application and documentation process.

The Colors Matter: The card color signifies the severity of the disability, which impacts benefit eligibility.
